Natures Youth Direct - Fax Application

To sign up via fax please read the Terms and Conditions and Policies and Procedures. Then print the Distributor Application, fill it out and fax it to 508-862-0190.

Distributors are allowed to order when they sign up, however if for any reason your application is not accepted by Nature's Youth Direct your order will not be processed.
Within 48 hours of receipt of your faxed or mailed application Nature's Youth will notify you of your application status.

Upon acceptance of your faxed application, your Distributor Information Packet will be mailed to you.
